If you do not see your plan listed above, please call the office to discuss self-pay rates. Please check back periodically as we are constantly updating our insurance providers. For individuals that are out of network you will be provided you a superbill that you can submit to your insurance company for re-imbursement.
Note: Being in-network does not guarantee coverage. Insurance coverage varies between individual pans as well as nutritional diagnosis. It is recommended that call your insurance company prior to your initial appointment to ensure your visit will be covered.
